Ingesting cocoa powder can cause symptoms like vomiting, heart attack, seizures and in severe cases even death. 

It is suggested that cocoa powder and chocolate products are never given to dogs as even the smallest quantities can be dangerous, depending on the weight of your pup. The ratio of theobromine to kilogram is 100-150 milligrams to one kilogram. Meaning if a 30 kg dog ingested 3000mg of theobromine, this could be a fatal dose.
